H-casa is based on a dual level of assistance, which intervenes on different targets: – a web app for citizens to use for a first informative self-diagnosis via questionnaire, establishing contact with the family doctor who, if appropriate, will contact him to perform a pre-triage by telephone; – a web portal for healthcare professionals that plans to analyze and categorize the information provided by citizens during pre-triage for the monitoring of symptoms, integrated support home care operations (e.g. tampon, drugs) and, consequently, to activate teleconsultation (audio and / or video) and telemonitoring; – diagnostic-instrumental devices for the transmission of clinical data from the patient’s home on the platform.
